Millions of Americans deal with problem areas such as love handles, arm fat, belly fat, and other stubborn places — even after reaching weight loss and fitness goals. Awake Liposuction in Dallas is a popular and effective cosmetic procedure that removes fat from targeted areas to help create a shapelier body and reach sought-after appearance goals. According to data from the International Society of Aesthetic Plastic Surgery (ISAPS), liposuction has become the most popular cosmetic procedure in the world.  Innovations Medical offers liposuction in our Dallas location.

The 1st doctor to offer awake tickle liposuction in the United States, Dr. Bill Johnson, talks on Good Morning Texas about why awake liposuction is the choice patients are asking for when it comes to liposuction surgeries.  It’s no wonder so many clinics have come up with new marketing names for the original Awake Liposuction.

What Is Awake Liposuction?

Awake liposuction is a minimally invasive fat removal procedure performed under local anesthesia — meaning you’re awake, alert, and comfortable during the entire process. This technique avoids the risks and recovery time associated with general anesthesia, making it a safer, faster alternative for body contouring. Awake liposuction represents the best advancement in body contouring – combining safety, effectiveness, and minimal downtime.

How Awake Liposuction Works

The procedure uses a special solution to numb the targeted areas, followed by gentle suction using vibration-assisted technology. At Innovations Medical, our approach minimizes bruising, swelling, and downtime — while delivering dramatic fat reduction and natural-looking results.

The doctor performing liposuction uses a cannula, a narrow tube with a blunt tip attached to a suction or vacuum device. This allows the doctor to:

  • Use the cannula and vacuum to remove fat from thousands of tiny holes made in the fat by the cannula. Think of it as making Swiss cheese out of the fat.
  • Leave the collagen web mostly intact to hold the remaining fat and overlying skin in place. Permanently remove the fat and the fat cells from the body.
  • Use further tools like SmartLipo to tighten the skin by shrinking and renewing the collagen network.
  • Reduce the size of the fat to give the area a slimmer, trimmer appearance. This occurs as the holes in the fat collapse.

Liposuction can be done on almost any area of the body with fat. Many different tools and types of procedures are used to produce beautiful results with liposuction. Injecting the tumescent fluid decreases swelling, bruising and the risk of infection.  That is why even liposuction under general anesthesia usually uses tumescent fluid. SO WHY HAVE THE EXTRA COST AND RISK OF GENERAL ANESTHESIA?

Tumescent Awake Liposuction Explained

Liposuction done solely under tumescent anesthesia is often referred to as tumescent liposuction and is also called “drive-through cosmetic surgery.” Originating from the Latin word for “swollen,” tumescent procedures involve pumping specially formulated fluid into the area until it becomes firm or swollen.

Tumescent liposuction offers several advantages over procedures involving general anesthesia:

  • Safer than techniques that require general anesthesia
  • Results are similar to or better than general anesthesia techniques
  • Allows the patient to return home on the day of the procedure
  • Lowers the cost of liposuction procedures by allowing them to be performed in the doctor’s office; there is no additional charge for a surgery center, hospital, or anesthesiologist

The tumescent fluid contains saline, lidocaine, sodium bicarbonate, and a form of adrenaline called epinephrine. This combination provides excellent numbing so liposuction and other procedures can be performed safely and comfortably. Specific safety benefits include:

  • Decreases swelling after liposuction procedures
  • Decreases bleeding and bruising after liposuction procedures because epinephrine constricts blood vessels and decreases bleeding and bruising
  • Decreases the risk of infection after liposuction procedures, as lidocaine inhibits the growth of bacteria
  • Provides extra fluids to the patient, reducing the dizziness and dehydration sometimes associated with liposuction

Almost all liposuction cases today add tumescent anesthesia to the treatment area because of the increased safety.

Areas Treated with Awake Liposuction

Awake liposuction can be used on nearly every part of the body where unwanted fat builds up. Popular areas include:

  • Abdomen (upper, lower, and sides — also called Lipo 360)
  • Thighs and hips
  • Upper arms
  • Back and bra rolls
  • Chin and jawline
  • Male chest (moobs or gynecomastia)

Types of Liposuction

There is a wide range of liposuction procedures, but not all are performed regularly by doctors. Common forms of liposuction are:

  • Suction Assisted Liposuction (SAL):  A simple cannula attached to suction. The physician supplies all of the power to remove the fat. This is commonly used in small areas like the neck or ankles.
  • Power-Assisted Liposuction (PAL):  Uses a mechanical device to move the cannula and help the physician remove fat. The power is usually provided by electricity or compressed air. Physicians using these devices can generally remove more fat and do more “sculpting” or shaping of the area. Care must be taken to avoid grooves or rippling.
  • Laser-Assisted Liposuction (LAL):  This adds a laser to the cannula. The lasers destroy fat cells and, when used correctly, can tighten the skin. SmartLipo was the first laser liposuction device available in the United States and remains the best-researched and widest-used. LAL should always be combined with suction to achieve the desired results. Always ask if your laser liposuction provider is adding suction to the laser. We also perform laser-based liposuction using SmartLipo. Dr. Johnson was the first physician in North Texas to use SmartLipo. SmartLipo provides twice the benefit: it melts away the fat and tightens the skin, allowing you to obtain that perfect figure you’ve always wanted. Dr. Johnson has performed more than 6,000 SmartLipo cases. We continually update our SmartLipo devices to keep up with advances in laser liposuction.
  • Ultrasound-Assisted Liposuction (USAL):  Uses ultrasound energy to heat and disrupt the fat. Vaser® is the most common USAL device in the United States. Vaser® has become known for high-definition liposuction, but the actual high definition is done with the suction device, typically a PAL device.
  • Vibration Assisted Liposuction (VAL):  A special form of PAL that uses compressed air to move the cannula in multiple directions. This makes VAL the most efficient liposuction device for removing fat. The vibration helps avoid the grooving and rippling common with PAL devices. VAL is the most popular form of liposuction in Europe, and several studies have shown it gives some of the best results. Vibro Lipo, called Tickle Lipo, allows providers to suction fat faster and smoother than previous technologies. Dr. Johnson was the first physician in the United States to offer this type of liposuction, making him a true leader and expert on the procedure. He has performed more than 5,000 cases using Vibro Lipo.
  • Radiofrequency Assisted Liposuction (RFAL):  Uses radiofrequency energy to help liposuction. Like USAL and LAL, RFAL must be added to suction or Power-Assisted Liposuction devices to achieve good results. RFAL is a good choice for skin tightening.

Advanced Skin Tightening with Smartlipo

At Innovations Medical, Smartlipo is an essential part of every awake liposuction procedure. Dr. Bill Johnson was the first physician in North Texas to offer Smartlipo in 2007, and we’ve used multiple generations of this technology ever since. After fat is carefully removed, Smartlipo laser energy is applied to gently tighten the skin from the inside, helping reduce laxity and improve final contour. This combination allows for smoother results, better skin retraction, and a more sculpted outcome—all while you remain awake and without general anesthesia.

What Is Lipo 360?

Lipo 360 is a term used to describe circumferential liposuction of the midsection — treating the abdomen, waist, flanks (love handles), and lower back in a single procedure to create a more balanced, contoured result from every angle.

Rather than focusing on one area alone, Lipo 360 addresses the full torso, which is why it has become one of the most requested body contouring approaches nationwide.

However, the term Lipo 360 is a marketing label — not a specific surgical technique. The results, safety, and recovery depend entirely on how the procedure is performed and what technology is used.

Awake Lipo 360 at Innovations Medical

At Innovations Medical, Lipo 360 is performed as an awake procedure under local anesthesia, eliminating the risks and downtime associated with general anesthesia.

Our approach combines:

  • Vibration-based liposuction to gently and efficiently remove fat

  • Smartlipo laser technology to support skin tightening after fat removal

  • Awake, outpatient treatment with same-day discharge

This method allows for precise contouring of the full midsection while minimizing trauma to surrounding tissues — a key reason patients experience faster recovery and less postoperative discomfort.

Dr. Bill Johnson was the first physician in the United States to offer vibration-based liposuction, and the first in North Texas to offer Smartlipo. These technologies have been integral to our awake body contouring procedures since 2007.

How Awake Lipo 360 Is Different From Traditional Lipo 360

Traditional Lipo 360 procedures are often performed under general anesthesia and may rely solely on suction-assisted fat removal. While effective, this approach can involve longer recovery times and higher surgical risk.

Awake Lipo 360 at Innovations Medical offers several important differences:

  • No general anesthesia

  • One outpatient visit

  • Faster return to normal activity

  • Skin tightening included for improved contour

  • Customized treatment based on body type and fat distribution

Because patients remain awake, safety is enhanced, and recovery is more predictable.

Who Is a Good Candidate for Awake Lipo 360?

Awake Lipo 360 is ideal for patients who want comprehensive midsection contouring and prefer a safer alternative to traditional liposuction.

Most healthy adults are candidates, including patients who:

  • Have stubborn abdominal and waist fat

  • Want full torso contouring rather than spot treatment

  • Prefer to avoid general anesthesia

  • Want a faster recovery timeline

During consultation, Dr. Johnson evaluates fat distribution, skin elasticity, and overall goals to determine whether Awake Lipo 360 is appropriate and which areas should be treated for optimal results.

Cost of Awake Liposuction

At Innovations Medical, awake liposuction typically ranges from $2,000 to $3,500 per area. The total cost depends on how many areas are treated and whether it’s combined with fat transfer procedures. We also offer 24-month 0% financing and options for patients with no credit.
