Awake Back Liposuction in Dallas — Bra Roll, Armpit and Upper Back
“Back fat” is really three different areas, and people often mix them up. Each one sits at a different height, is treated differently, and is planned differently. Knowing which one bothers you makes your consultation faster and your quote clearer.
Every back area is treated with awake liposuction under local anesthesia, so you stay comfortable and alert, without general anesthesia or a hospital stay. Innovations Medical has offered awake liposuction in Dallas since 2007, and Dr. Bill Johnson and our team have completed 6,000+ awake liposuction cases.

Which back area is yours?
Stand in front of a mirror in a fitted top or bra and look from the side and the back.
| Area | Where it is | What you notice | How it’s usually planned |
|---|---|---|---|
| 1. Mid back (bra roll) | At bra-strap level, around the middle of the back | A roll above or below the strap; lines through fitted tops | The most common back area. Usually done together with Lipo 360 (abdomen + flanks) |
| 2. Axilla (armpit fat) | Above the bra roll, at the front and side of the armpit | A pocket that pushes out beside a sleeveless top or strap | Often paired with arm liposuction |
| 3. Upper back | The highest area, across the shoulder blades toward the base of the neck | A thick, padded upper back or a “hump” area; tight-fitting shirts across the shoulders | Flat $3,500; can be combined with other areas the same day (one upper back case per day) |
Not sure? Send photos or bring them to your consultation. Dr. Johnson will show you exactly where each area begins and ends on your body.
1. Mid back (bra roll)
The mid back is the area most people mean when they say “back fat”: the soft rolls right at bra-strap level that show through fitted tops even when the rest of the body is fit. This fat is dense and fibrous, closely attached to the skin, which is why it shows up as distinct rolls and doesn’t respond to rows, pull-ups or spot training.
Why it’s usually done with Lipo 360: the bra roll continues into the flanks and waist. Treating the mid back together with the abdomen and flanks, our Lipo 360 awake liposuction, gives a smooth, continuous torso from every angle instead of a slimmer waist with a roll still sitting above it.
Pricing: $3,500 as a single area, or $2,500 when added to other areas. The mid back is almost always added to Lipo 360 or another liposuction plan.
Mid back FAQ
Can I treat just the bra roll? Yes, at $3,500 as a single area. Added to other areas it’s $2,500. Many patients add it to Lipo 360 because the areas blend together, and Dr. Johnson will recommend what gives a balanced result. Will exercise fix a bra roll? Exercise strengthens the muscle underneath and lowers overall body fat, but it can’t target this fibrous layer. Read how to reduce bra fat. Where are the openings? Usually placed under the bra line, where they’re easy to hide.
2. Axilla (armpit fat)
Axillary fat sits just above the bra roll, at the front and side of the armpit. It’s the small pocket that pushes out next to a strap or a sleeveless top. It’s not breast tissue and not upper-back fat, though it’s often mistaken for both.
Because it sits where the arm meets the chest, axilla liposuction is frequently paired with awake arm liposuction for a clean line from the upper arm into the side of the chest.
Pricing: $2,500.
Axilla FAQ
Is armpit fat the same as bra fat? No. Armpit fat sits higher and further forward, at the front of the armpit. Bra fat sits on the back at strap level. Can I combine axilla and arms? Yes. It’s one of the most common pairings, because the two areas form one contour. Is it breast tissue? Usually it’s fat. Dr. Johnson will examine the area, and if anything looks like tissue that needs other evaluation, he’ll tell you.
3. Upper back
The upper back is the highest area: across the shoulder blades toward the base of the neck. Fat here is especially thick and fibrous, and removing it smoothly is physically demanding, careful work.
Pricing: upper back is always $3,500. It’s a full price on its own, because of the time and effort the area requires.
Planning note: because of that workload, Dr. Johnson books only one upper back case per day. If you’re planning around time off work, travel or an event, it’s worth asking about available dates at your consultation so you can pick the day that suits you.
Upper back FAQ
Why is the upper back priced separately? The tissue is dense and the area takes significant time and effort to treat well, so it’s priced as a full procedure at $3,500. Why only one per day? It lets Dr. Johnson give each upper back case his full attention and energy. It’s simply something to factor into your scheduling. Can I combine the upper back with other areas? Yes. The upper back can be done on the same day as other areas. The one-per-day limit only applies to upper back cases.
Liposuction or a lift? (Candidacy for every back area)
Liposuction removes fat, not loose skin.
You’re likely a good candidate if: – The area is mostly pinchable fat, not hanging skin – Your skin springs back when gently pinched – You’re at or near a stable weight, in good health, and don’t smoke (or are willing to stop)
A back lift may be more appropriate if: – You’ve had major weight loss, including bariatric surgery or significant GLP-1 medication weight loss, and have loose, overhanging folds – The rolls stay even when you lie flat, with very little fat to pinch
A back lift removes skin through a long incision, usually under general anesthesia, and isn’t one of our awake procedures. Some patients with mild laxity see improvement with laser-assisted liposuction, but results vary, and Dr. Johnson will give you an honest opinion.
How the awake procedure works
- Marking: you stand while we mark each area exactly as it appears in your clothes.
- Numbing: tumescent local anesthesia. You’re awake and can talk with the team, and oral comfort medication is available.
- Fat removal: fine cannulas through tiny openings. Because you’re awake, you can change position so symmetry can be checked.
- Home the same day in your compression garment, with a companion.
Recovery
- First 48 hours: drainage, swelling and soreness similar to a hard workout
- Days 2–5: most patients return to desk work; wear compression and walk daily
- Weeks 2–4: bruising fades; light exercise as cleared
- Months 1–3: swelling settles; the final result is typically assessed around 3–6 months
The upper back can feel stiffer for the first several days because of how dense the tissue is. Garments are purchased separately: see our recommended compression garments and our compression garment guide.
Pricing summary
- Upper back: $3,500 (one case per day)
- Mid back (bra roll): $3,500 as a single area; $2,500 when added to other areas (almost always added, usually to Lipo 360)

- Includes local anesthesia and the in-office procedure, with no anesthesiologist or hospital fee
- Compression garments are not included; they’re purchased separately (recommended options)

General back FAQ
Does back liposuction hurt? The area is numbed with local anesthesia. Most patients feel pressure and vibration rather than pain, and soreness afterward. Will the fat come back? Removed fat cells don’t regrow. Keeping your weight stable protects your result. Will I have scars? The openings are a few millimeters wide, placed where they’re easy to hide, and typically fade over time.
